Please contact my office at 972.200.4798 for information regarding fees, insurance coverage, and other financial inquiries.

Payments are due at the time of service. Payments may be made by cash, check, or credit card.

Insurance: I am an in-network provider for insurance plans including Blue Cross, Aetna, and Tri-West. I also am a provider for several Employer Assistance Programs (EAP). Please contact the office for more information regarding in and out-of-network coverage.

Cancellation Policy

If you miss your appointment or cancel your appointment less than 24 hours prior to the appointment time, you will be billed my regular non-insurance session fee for the missed appointment . Please note that insurance providers do not reimburse for missed appointments. To cancel or reschedule your appointment, you may contact me by phone or email.
